Why Failure Analysis doesn't work - and How to Fix it?

In projects with high software quality and safety requirements, engineering teams often face three major challenges - meeting project deadlines, producing the required artifacts and ensuring genuine system reliability. Various types of failure analyses are used to improve system reliability and support this objective. However, in real practice, failure analyses often turn into collection of "dead" artifacts that do little to make your software more reliable or predictable. Instead, they tend to burden development processes more difficult, creating a stream of low-value activities - reducing the speed of development and team motivation.
In this report, I will share my experience in implementing and applying failure analysis in software development and show you how the process is built. Let's discuss how the Docs-as-Code approach allows you to keep the analysis results up-to-date and traceable in the face of constant system changes without significantly increasing labor costs.
